Authorization For Release of Information (PDF)
You can request that Quello Clinic, Ltd. release medical record information to another party by printing and completing the online "Authorization for Release of Information" form. Ensure all form fields are filled out completely. Each section requires completion to accurately process your request. Please sign the completed form and fax, mail or drop off at any Quello Clinic location. There may be a fee if request is for other than continuing care.
